Tesamorelin is a 44-amino-acid synthetic peptide derived from native GHRH with a trans-3-hexenoyl group attached to the N-terminal tyrosine. The modification protects the peptide from DPP-4 cleavage and produces a substantially longer functional half-life than unmodified GHRH(1-29) or sermorelin.

Published preclinical and clinical research has explored tesamorelin in studies of visceral adipose tissue, lipid metabolism, IGF-1 axis modulation and the broader growth-hormone secretory pathway. It remains the gold-standard reference GHRH analogue in laboratory work.

Lyophilised tesamorelin is supplied as a sterile white powder under inert atmosphere. Hold unopened vials at 2–8 °C; transfer to −20 °C only if reconstitution will not happen within 8 months. Reconstitute with bacteriostatic water for a two-month (60-day) in-use stability window under refrigeration; minimise light exposure to preserve the N-terminal tyrosine.

Most laboratories favour 2–5 mg/ml stock concentrations for ease of microlitre dispensing. Tesamorelin is sensitive to repeat freeze–thaw cycles — aliquot the full reconstituted volume before any freeze to preserve potency.

Researchers often compare tesamorelin with CJC-1295 (with and without DAC) and ipamorelin in growth-hormone secretory-axis panels. Tesamorelin is a stabilised GHRH analogue; CJC-1295 is a 30-residue GHRH analogue with optional DAC (drug-affinity-complex) modification for extended half-life; ipamorelin is a selective ghrelin/GHSR receptor agonist that acts on a different upstream pathway entirely.
